So you just slapped the heads back on again? I can't say I would have done that. Probably be doing gaskets again sometime soon. The gaskets fail from the heads not being flat and the TTY bolts stretching.
When it smokes, try and smell it and see if it smells like fuel or coolant.
Some more info about the mods on your truck would help too. Try filling out your sig so we know what you're working with here. Have you tried putting it back to stock tuning (assuming you're running some sort of programmer?) to see what happens?

It's tough helping out in this situation with the limited information given. Questions have been asked and not answered as well. We really can't help much out with out more info.
Do you have access to a scanner of some sort to monitor some things? An injector might have loosened back up causing it to run crappy. Tough to say.
