Set tire pressure to max indicated on the tire sidewall, check/replace air and fuel filters, remove unnecessary weight, drive like a granny, get alignment and shocks,suspension components checked, etc. I'm probably forgetting some, cover your basics, then dig deeper with mods.

Right, do the basics first as stated, even check the brake calipers to make sure they are operating properly and not hanging up. A big problem with the later trucks is the programming Ford put on them. getting the early tunes back on will help alot, custom tunes will help even more. Getting rid of the stock exhaust and going with at least a 4" , you can use performance muffler that comes with a kit, they flow pretty good. getting rid of cat alone is probably worth up 2mpg. Delete the EGR cooler, worth another 1-1 1/2mpg. Synthetic oil, worth maybe another 1-1 1/2 mpg when you use synthetic gear oil in diffs too. with 373 gears you should be able to get 18mpg pretty easy, a big problem alot of people have is when they tune a truck it runs SOOOOOOO much better it is hard for them to keep their foot off the skinny pedal causing them to loose all the advantage of good mileage.

no mods done at all stock truck 2006 i have no idea about gears looking for best replacement for the air intake and anything else i can do to improve

The stock air intakes are really good on these trucks, there is a mod you can do to get more air to the stocker, think its called the "zoodad" mod. You will notice alot more gains from a good turbo back exhaust, with no cat converter. SCT seems to be they going rage right now with the tuners, if you tow it would be highly recommended. It is best to get a tuner like the SCt from someone that makes custom tunes for them like Innovative Diesel, you can call them and tell them what you want, like a tow tune, mileage tune, power tune, whatever you want, I believe you can get several tunes on one controller so you can choose what will be best for the driving conditions you expect.

Shoot I have done all this and get 15 and drive like.....well i stole it most of the time but even in granny mode i get 15 or towing the dakota on the trailer so im not convinced it has much to do with staying off the skinny pedal. I think most of has to do with kepping boost below about 15psi and above 5psi. But thats just my opinion.

More boost causes the programming to inject more fuel. Bruce Malliison of Pittsburgh Power wrote that rpm doesn't burn fuel, boost does. So instead of lugging your engine and building high boost,downshift a gear, let the engine rev under less strain. Or at least get an accurate boost gauge and keep keep an eye on it.
