Default bad bucking and missing

Alright guys so ive had a few of my friends look at my 05 f250 6.0l 169000 miles pretty much all stock except for egr delete and sct tuner with tunes from innovative diesel. Its been a few weeks ive been trying to figure it out its almost feels like a miss but if i push the peddle down it works through it and ***** and gets then it will start doing it again other times it will shake or buck so much that my ashtray full of change will start flinging coins all over the floor, first i thought it was an injector going out due to the sluggish/missing feeling and some white smoke at start up when its cold and i dont have it plugged in(doesnt smoke if plugged in) so i did and oil change and put some hot shot secret in drove it for a few days bout 300-400 miles and still really rough so had a certified diesel mechanic check the injectors he said they are good and my head gaskets are good he works for my friend but he didnt have alot of time to mess with my truck because they have 6 powerstrokes that he is always doing something to, but i did have him check for codes and theres no codes. So my other buddy knows a gas mechanic which i know i shouldnt have wasted my time with his advice but he told me sounds like possibly a camshaft sensor so i figured for the price that they cost i would go ahead and try it well that didnt do **** except cost me alittle money and make me cold from laying outside replacing it. My next thought was possibly the icp sensor ive heard that can make those type of symptoms but ive never dealt with that before so just wondering if anybody else has had this type of issue or if yall think it sounds like the icp sensor or something else. Im pretty much out of ideas i checked the ficm over the summer it tested fine then but i havnt checked it since ive had these problems.

I had an injector that had the little/tiny/ bolt come loose that holds the pizeo magnets and it would rattle your teeth. but if you really stepped on it it would run great but it was definitely realy bad at 55-60mph, at 75 it wasn't too bad. I pulled the injector out and put Loctite on the nut and never had another problem out of it.
