mad .6.0 no start (typical issue, not so typical details)

I have a 2006 F-350 6.0 PSD. Bought the truck with a blown HG, had 170k miles all the clock and all stock (no "bulletproofing) had evidence of previous tuner.

Long story short, I pulled the motor and completed;

Head Gaskets
Check/Deck heads
ARP Studs
Oil Cooler
EGR Delete
STC Fitting
SP/Dummy Plugs
Reseal injectors
Reseal hpop, IPR, and all other top end seal/gaskets

After reassembly and instillation I got the truck running, took on first trip and after about a mile;

Truck started black smoking badly then lost power. Low/No boost code was thrown, then she died. Went to restart and it had hydro locked itself. Pulled it back to the house and yanked passenger side glow plugs and found the locked cylinder. Drained it and it was filled with fuel. Removed injector to find the hold down was very loose so I believed I hadn't torqued it down. After inspecting the injector I noticed the copper washer was mushed flat as a piece of paper so either (I torqued far more than I remember, or the cup wasn't seated and when the oil rail was torqued it caused it to flatten so much?)

Resealed injector, put it together. TRUCK RAN PERFECT. for 3 days...

Went to the store and shut her down, came back out, no start. Crank. crank. crank.

Now to the current issue. At first when cranking the ICP read 250-300psi, and while cranking IPR % 84. (something)

Let it cool down over night to see if hot start issue and after a few minutes of cranking I'm at ZERO psi on my ICP. IPR% still shows 84 point something.

Took my air hose and put it on ICP port to pressure check and under the PASSANGER (SAME SIDE) valve cover I have a whole LOT of air noise coming out. (explained ZERO psi)

For the record I did two tests on IPR to see if it was the culprit (sticking open).
first was pressurize system and give the IPR 12v direct and see if noise gets louder (IPR closing) and it did. Next just to be sure I pulled the IPR and blew in it, very difficult but air would go through, applied 12v and blew in it, no air went through. So It is opening/closing.

I've attempted to pressure test the oil log by itself, though it is difficult to keep all the things plugged on it while applying air. Seems to have passed, all oil ***** and O rings looked good, no obvious cracks or holes. Injectors all look good, no signs of obvious issue, so what do I do or try now?
